Client Rights and Complaint Information

In accordance with Texas Health and Safety Code §181.105, LifeWise Counseling and Wellness, LLC provides the following information regarding access to health care records and options for filing a complaint. Texas law requires covered entities to provide consumers with information about requesting records and contacting relevant complaint authorities. (Texas Statutes)

Requesting Your Health Care Records

Requests for health care records may be submitted in writing.

You will be required to complete and sign an authorization or records request form. Requests will be processed in accordance with applicable Texas and federal privacy laws.

Texas Behavioral Health Executive Council

Concerns involving the professional conduct of a Texas licensed behavioral health provider may be submitted to the Texas Behavioral Health Executive Council. Complaints must be submitted using the Council’s approved complaint form. (BHEC)

Texas Attorney General Consumer Protection Division

If you believe you have experienced a false, misleading, deceptive, or unfair business practice, you may submit a consumer complaint to the Texas Attorney General’s Consumer Protection Division. (Texas Attorney General)
