LifeWise Counseling and Wellness, LLC is committed to providing clear, ethical, and accurate information about our services, professional roles, privacy practices, and client rights.
Professional Credentials
Stacy Hixon, MA, LPC-S, CCTP, FRTP, is a Texas Licensed Professional Counselor Supervisor.
Texas LPC License #77727
Stacy is currently the only independently licensed professional counselor on the LifeWise care team. Other care team members provide coaching, administrative, educational, or supervised services according to their training, credentials, and authorized roles.
Life coaching is separate from counseling and psychotherapy. It does not include mental health diagnosis or treatment.
Texas rules require counseling advertising to accurately identify licensure status. LPC Associates must clearly identify themselves as Associates and disclose their board approved supervisor. (THECB)
Location of Counseling Services
Counseling services are provided only when the client is physically located in Texas at the time of the session.
In person counseling is available in North Fort Worth. Secure telehealth services are available throughout Texas.
Privacy
LifeWise Counseling and Wellness, LLC protects client information according to applicable privacy laws and professional ethical standards.
Our Notice of Privacy Practices explains how protected health information may be used, disclosed, accessed, and corrected, as well as how clients may submit a privacy complaint. Covered health care providers must provide this notice, and current federal model notices are available through the U.S. Department of Health and Human Services. (HHS.gov)

Please do not submit detailed clinical, medical, or emergency information through a general website contact form.
Submitting a form, email, or text message does not establish a therapist client relationship. Services begin only after the appropriate intake, informed consent, and scheduling process has been completed.
Good Faith Estimates
Clients who are uninsured or who choose not to use insurance may request a Good Faith Estimate of expected charges before receiving services.

Crisis Support
LifeWise Counseling and Wellness, LLC is not a crisis service, and this website, email, and text line are not monitored continuously.
