In compliance with Texas Health and Safety Code Section 181.105, the following information is provided to help you access your records, contact licensing authorities, or file a complaint if needed.

Requesting Your Health Care Records

If you would like to request a copy of your health care records, please contact our Care team:

Requests may be made in writing. You will be required to sign a release of records. We will respond in accordance with Texas privacy and confidentiality laws.

Filing a Complaint with the Office of the Attorney General

If you believe you have experienced unfair or deceptive practices you may file a consumer complaint with the Texas Attorney General’s Consumer Protection Division:

  • Unfair practices: Actions that take advantage of or harm you – for example, being charged for services you didn’t receive.
  • Deceptive practices: When you’re misled or given false information – for example, a counselor claiming credentials they don’t have.
